Tesamorelin is a synthetic peptide analog of growth hormone-releasing hormone (GHRH). Its sequence corresponds to the 44-amino-acid form of human GHRH with a trans-3-hexenoyl group attached to the N-terminal tyrosine. This modification slows enzymatic cleavage and extends the peptide's activity relative to the native hormone. The compound is produced by solid-phase peptide synthesis and supplied as a lyophilized powder. Researchers classify it as a GHRH receptor agonist. Its structure places it in the same family as other growth hormone secretagogues that act on the pituitary.
Binding of tesamorelin to GHRH receptors on pituitary somatotroph cells triggers cyclic AMP signaling and the release of growth hormone into circulation. Because the peptide acts upstream of the growth hormone axis, its effects are partly mediated by hepatic insulin-like growth factor 1 (IGF-1) production. The pulsatile character of endogenous growth hormone secretion is preserved rather than replaced. Whether amplified signaling produces effects beyond those of native GHRH remains an area of ongoing investigation.

Physicochemical behavior is dominated by the peptide backbone. The molecule is hydrophilic and carries a net positive charge near neutral pH, owing to several arginine and lysine residues. In solution it adopts a largely unstructured conformation, and aggregation is a known concern for peptide products of this size. Oxidation of methionine and deamidation of asparagine or glutamine residues are the principal chemical degradation routes. These liabilities shape how the material is formulated, handled, and analyzed, and they explain why lyophilized presentations are common in research settings.
Tesamorelin is a synthetic peptide analog of growth hormone-releasing hormone, built from 44 amino acids. Its sequence follows the natural human GHRH(1-44) backbone, with a trans-3-hexenoyl group attached to the N-terminal tyrosine. This modification blocks recognition by dipeptidyl peptidase IV, the enzyme that rapidly truncates the native hormone in circulation. The result is a molecule with a substantially longer plasma residence time than unmodified GHRH, which makes it practical for clinical and laboratory study.
Receptor-level activity begins when the peptide binds the GHRH receptor, a class B G-protein-coupled receptor found on pituitary somatotroph cells. Occupancy triggers Gs-mediated activation of adenylyl cyclase and a rise in intracellular cyclic AMP, which in turn promotes synthesis and pulsatile release of growth hormone. Because the compound acts upstream of the growth hormone axis rather than supplying hormone directly, its effect depends on intact pituitary function. Binding studies in cell culture and animal models have established this pathway; the detailed kinetics of receptor recycling in humans remain less well characterized.
Clinical study of tesamorelin has centered on adults with HIV-associated lipodystrophy, a condition in which abdominal fat accumulates while peripheral fat is lost. In controlled trials, treated participants showed reductions in visceral adipose tissue measured by imaging, alongside modest shifts in some lipid values. Effects on subcutaneous fat were smaller and less consistent across studies. Whether these changes translate into fewer cardiovascular events remains an open question, because the trials were not designed or powered to answer it.
Tesamorelin is a synthetic peptide that acts as an analog of growth hormone-releasing hormone, a natural hypothalamic signal. Its sequence corresponds to the forty-four amino acid form of the human hormone, with a small acyl group attached near the amino terminus. That modification slows enzymatic breakdown and extends the time the peptide remains active in circulation. The compound was developed as a pharmacological way to raise endogenous growth hormone output rather than supplying the hormone directly.
=== Notable people === The American pop singer Michael Jackson had the condition, which he obscured with costumes and makeup until it covered his body. This led to some inaccurate speculation that he had bleached his skin. Other notable cases include the American rapper Krizz Kaliko, the Canadian fashion model Winnie Harlow, the New Zealand singer-songwriter Kimbra, the American actor David Dastmalchian, the Argentine musician Charly García, the professional wrestler Bryan Danielson, the French actor Michaël Youn, the former French Prime Minister Édouard Philippe, Miss Universe Egypt 2024 Logina Salah, governor of Pampanga Eddie Panlilio, and the Colombian model Taliana Vargas.
== Treatment == In most of the reported cases, the treatment options were very similar. Plasmapheresis alone or in combination with steroids, sometimes also with thymectomy and azathioprine, have been the most frequently used therapeutic approach in treating Morvan's Syndrome. However, this does not always work, as failed response to steroids and to subsequently added plasmapheresis have been reported. Intravenous immunoglobulin was effective in one case. In one case, the dramatic response to high-dose oral prednisolone together with pulse methylprednisolone with almost complete disappearance of the symptoms within a short period should induce consideration of corticosteroids. In another case, the subject was treated with haloperidol (6 mg/day) with some improvement in the psychomotor agitation and hallucinations, but even high doses of carbamazepine given to the subject failed to improve the spontaneous muscle activity. Plasma Exchange (PE) was initiated, and after the third such session, the itching, sweating, mental disturbances, and complex nocturnal behavior improved and these symptoms completely disappeared after the sixth session, with improvement in insomnia and reduced muscle twitching. However, one month after the sixth PE session, there was a progressive worsening of insomnia and diurnal drowsiness, which promptly disappeared after another two PE sessions.
== History and synthesis == DMF was first obtained in 1893 by the French chemist Albert Verley (1867–1959), by distilling a mixture of dimethylamine hydrochloride and potassium formate. It is now industrially manufactured by combining methyl formate and dimethylamine or by reaction of dimethylamine with carbon monoxide. Although currently impractical, DMF can be prepared from supercritical carbon dioxide using ruthenium-based catalysts.

Humans have more than the commonly cited five senses. The number of senses in various categorizations ranges from 5 to more than 20. In addition to sight, smell, taste, touch, and hearing, which were the senses identified by Aristotle, humans can sense balance and acceleration (equilibrioception), pain (nociception), body and limb position (proprioception or kinesthetic sense), and relative temperature (thermoception). Other senses sometimes identified are the sense of time, echolocation, itching, pressure, hunger, thirst, fullness of the stomach, need to urinate, need to defecate, blood carbon dioxide (CO2) levels, and electric field sensation. All different tastes can be detected on all parts of the tongue by taste buds, with slightly increased sensitivities in different locations depending on the person; the tongue map showing the contrary is fallacious. There are not four primary tastes, but five: in addition to bitter, sour, salty, and sweet, humans have taste receptors for umami, which is a "savory" or "meaty" taste. Fat does interact with specific receptors in taste bud cells, but whether it is a sixth primary taste remains inconclusive. The human sense of smell is not weak or underdeveloped. Humans have similar senses of smell to other mammals, and are more sensitive to some odors than rodents and dogs.
Watch manufacture was becoming streamlined; the Japy family of Schaffhausen, Switzerland, led the way in this, and soon afterwards the newborn American watch industry developed much new machinery, so that by 1865 the American Watch Company (afterwards known as Waltham) could turn out more than 50,000 reliable watches each year. This development drove the Swiss out of their dominating position at the cheaper end of the market, compelling them to raise the quality of their products and establish themselves as the leaders in precision and accuracy instead.
The two known blockers which are specific to P-type calcium channels are peptides derived from the spider venom of Agelenopsis aperta. The toxins from this venom which show selectivity for P-type channels are ω-agatoxin IVA and ω-agatoxin IVB. Each of these peptide toxins are made of 48 amino acids which are bound by four disulfide bonds. Although ω-agatoxin IVA and ω-agatoxin IVB have the same affinity and selectivity for P-type channels, their kinetics are different. The ω-agatoxin IVA effects the gating mechanism of the P-type channel. When there is a strong depolarization to activate the channel, ω-agatoxin IVA can no longer block the channel. Therefore, ω-agatoxin IVA has a very low affinity for the channel when it is open. It binds to the α1A subunit on the outside of the pore. The ω-agatoxin IVA receptor on the P-type channel is located at the S3-S4 linker. On the other hand, channel blocking by ω-agatoxin IVB occurs much more slowly. Yet, similar to ω-agatoxin IVA, ω-agatoxin IVB cannot bind to the channel upon a strong depolarization.
An increase in the expression of Group 2 mGluRs, which could arise from a chronic under stimulation of these receptors, has been associated with schizophrenia. An increase in levels of system Xc- has also been found in postmortem schizophrenia patients, indicating that there may have been a decrease in net function of these receptors as well, leading to greater expression. It has been observed that Schizophrenia patients have a decreased level of glutathione in their prefrontal cortex, further supporting the conclusion that system Xc- may not be functioning properly. Clinical trials have shown therapeutic potential for N-acetylcysteine in treating schizophrenia. Again, changes in EAATs due to disruptions in Glutamate homeostasis may also be involved. Recent study showed that mRNA expression levels of both SLC3A2 and SLC7A11 in WBCs of schizophrenia patients are lower than that of healthy individuals. The finding supports the hypo-glutamatergic neurotransmission hypothesis in schizophrenia.
The reaction proceeds in two stages. The first protein component, called RebP, is an oxidase which contains heme and uses oxygen and nicotinamide adenine dinucleotide (NADH) to form the new aromatic bond between the indole components, making a six-membered ring. The RebP component then acts with a flavin-dependent partner called RebC to remove the two carboxylic acid groups by oxidative decarboxylation.

It mirrors the 44-residue form of human growth hormone-releasing hormone. A hexenoyl group on the N-terminal tyrosine distinguishes it from the unmodified hormone. The change is intended to improve resistance to enzymatic breakdown.
The N-terminal modification reduces cleavage by circulating peptidases, so the peptide persists longer than native GHRH. That persistence is the main rationale for the synthetic design. Comparative half-life values in humans are reported in regulatory review documents rather than in general reference literature.
Reductions in visceral adipose tissue have been measured in controlled studies of defined populations. Whether the effect generalizes to other groups and persists after treatment stops is less clear. Longer-term outcome data remain limited.
